Inspection Station License, Official Business Certificate

Inspection stations inspect motor vehicles for compliance with safety and emissions regulations. This certification is issued by the New York State Department of Motor Vehicles (DMV).

a completed Original Facility Application;
a Sales Tax ID number and copy of your Sales Tax Certificate of Authority;
proof of Worker's Compensation Insurance;
a photocopy of photo ID for all listed on the application;
receipts for your purchase of the inspection station sign;
all required fees; and
a copy of your Certificate of Assumed Name ("Business Certificate").
